Telehealth monitoring allows providers to effectively manage the health of individuals remotely through the use of an electronic device or monitor. Individuals have the ability to take their own vital signs at any time of day in the comfort of their own residence. In addition, the users will answer a series of questions regarding their current condition that are customized to their specific chronic disease. The patients’ responses complement the vital sign data to help provide an accurate assessment of their current health status. The data is then transmitted via a phone line to a local monitoring station and analyzed by registered nurses (RNs) and paramedics.

With Easy-To-Use Monitoring Devices, Telehealth Participants Can:
• Measure blood pressure
• Measure blood oxygen
• Measure weight
• Measure pain level
• Measure peak flow
• Record temperature
• Record blood glucose
• Record PT/INR results
• Record fluid level
